Authorization and Acknowledgement: By signing this waiver and consent, I, as legal parent/guardian grant full permission for my child(ren) to participate in any and all ARS Summer Camp (Camp) program activities set for the duration of the Camp, including but not limited to weekly trips, recreational swimming excursions, indoor and outdoor sports activities, neighbourhood walks, visits to neighbourhood parks, and other activities not listed. I understand that participation in these activities includes some inherent risk and accepts all risks and dangers therein.
I certify that my child(ren) is/are fully capable of participating in gym and playground activities and that my child is healthy and has no physical disabilities that would restrict him/her from fully participating in such activities.
Expectations of the Participant Camper(s): Campers are expected to abide by the rules and regulations of the Camp at all times as imposed by the Camp counselors and staff. Campers must be kind and respectful to everyone (campers and staff). Violence is not tolerated on the premises of the camp (hitting, pinching, kicking, etc). Any camper who performs physical attacks such as kicking, hitting, pushing, verbal or written threats that express an intent to harm, will not be accepted to attend camp for the rest of the summer.
Photography/Videography Release: I hereby authorize the Camp to photograph and/or otherwise record images, film and/or sounds of, or including my child(ren) while he/she participates in Camp activities. This includes recording, film, photographs, audiotape and/or videotape, and my child(ren)’s name and image, appearance, performance or voice. I hereby grant permission for the Camp to use the above-mentioned recordings, images or likenesses by publishing, posting on the Camp website or within the Camp, and/or broadcasting on television or radio for the purposes of Camp promotion and exhibition, including publication and/or distribution of such materials in print or electronically. I understand that no royalty fee or any other compensation of any kind shall become payable to me by reason of such release and use of any photograph, image or recording.
Medical Consent: In the event of a medical emergency requiring immediate medical attention, I give permission for medical assistance to be given to my child. Furthermore, if I cannot be reached and emergency care is needed, I give permission for the responsible person at the Camp to make arrangements for my child(ren) to be transported to the nearest hospital emergency room. The hospital has my authority to provide my child(ren) with all medical care they deem necessary, and I agree to be financially responsible for any and all care given.
Release of Liability and Indemnity: I agree to indemnify and hold harmless the Camp, staff, counselors and other employees from all claims for injuries, damages, losses, costs, expenses, actions and causes of action of all kinds, including legal fees, in any way arising from or related to my child(ren)’s involvement in Camp activities, both on Camp premises and at other sites owned or operated by third parties (i.e. for field trips).
Parent Guide: I have read and understand all policies and procedures set out in the “Parent Guide” including the late pick up policy.
